Director, Provider Implementations

About Stellar Health:

Historically, US Healthcare has relied on a fee-for-service reimbursement system where providers are paid based on the quantity of patient visits and procedures, rather than the quality of health outcomes.

At Stellar Health, we help primary care providers put patient health first. Our platform - a mix of technology, people, and analytics - supports providers at the point of care, delivering real-time patient information, activating practice staff, and empowering providers and care teams with incentives that reward the work they are already doing to keep patients healthy. Using the Stellar App, our web-based, point-of-care tool; practices receive a simple checklist of recommended actions that support the best quality care. Providers and care teams are then paid monthly for each action they complete, and Payors save money in reduced healthcare costs along the way.

Stellar is a US-based Health-tech backed by Top VCs (General Atlantic, Point72, & Primary Venture Partners) with an established product & proven operating model. We’ve shown that we make a real difference for physician practices and their patients.

About the position:

Stellar Health is at a pivotal growth point. We have a proven product, an established operating model, and a high-performing Provider Implementations team that is operational. We are seeking a Director of Provider Implementations to take this foundational success and architect the next phase of our scaling engine.

Reporting to the COO, you will lead and mentor a team of Implementation specialists. This is not a "build from scratch" role; instead, you will focus on optimizing existing workflows, closing data gaps to drive better insights, and ensuring our implementation programming evolves to meet the needs of increasingly complex medical groups.

You will be the strategic bridge between Network Growth, Product, and Provider Performance leadership, ensuring that our implementation process remains a competitive advantage. The ideal candidate is a systems-thinker who is equally comfortable de-escalating a high-stakes executive conversation as they are auditing a data pipeline. You will be responsible for maintaining our high-energy, hybrid team culture while ensuring we hit the aggressive performance targets required for the next stage of Stellar’s growth.

While many implementation roles are purely technical, Stellar’s implementation is rooted in clinical operational workflow. You will lead the team responsible for taking medical group handoffs from the Sales/Network Growth team and ensuring they are set up to perform successfully on the Stellar platform with ongoing practice support from the Provider Performance team.

Over your first 6 months you will:

Be a trusted leader for a great team - You’ll have moved past the learning phase and built real relationships with your direct reports. You won’t just be managing their tasks; you’ll be their advocate, helping them navigate their own career growth at Stellar while keeping the team’s day-to-day operations running smoothly.

Have the 2026 roadmap locked and loaded - You’ll have a deep enough understanding of how we work today to know what needs to change for tomorrow. You’ll have defined the "V2" of our implementation program - prioritizing the big swings we need to take in 2026 to make sure our provider groups aren't just "live," but are actually hitting their performance targets and earning incentives.

Move us from tracking "tasks" to tracking "momentum." You’ll have spotted the gaps in our current data and filled them. Instead of just knowing if a practice finished a checklist, we’ll have a clear, automated view of practice health. You’ll be using these insights to tell a story to the rest of the business about where we’re winning and where we need to pivot.

Find and fix the friction in our "scaling machine" - You’ll have identified the specific bottlenecks that slow us down as we grow. Whether it’s a handoff issue with Sales or a repetitive manual step in clinical setup, you’ll have implemented at least two major improvements that make the process more efficient for the team and more effective for our providers.

Be the "strategic foresight" for the department. You’ll have your eyes on the horizon, anticipating risks in the 2026 and 2027 pipeline before they become fires. You’ll be the person we look to for a proactive plan on how to maintain our high standards of customer success as we continue to scale.

What you'll do:

Team Leadership & Development: Build and scale a high performing org. Directly manage and mentor a team of Implementation Specialists and leads; drive team execution and professional growth.

Strategic Scaling: Apply systems thinking to evolve our "implementation machine," turning manual processes into scalable, repeatable operations that support rapid company growth.

Post-Sales Leadership: Partner closely with Network Sales to manage post-sales operations, ensuring a seamless transition for new large medical group onboardings.

Drive Strategic Cross-Functional Influence: Act as the primary bridge between Network Growth, Product, and Provider Success Performance teams. You will influence the Sales pipeline by defining operational feasibility and partner with Product to ensure the 'voice of the provider' is translated into features that automate manual implementation hurdles.

Architect Clinical Workflows: Serve as the chief designer for our implementation playbooks. You will identify where our current workflows are "leaky" and design the V2 strategies that ensure medical groups adopt the Stellar platform as a core part of their daily clinical operations.

Risk Mitigation & Foresight: Anticipate downstream operational risks in the onboarding pipeline and implement preemptive solutions before they impact client satisfaction.

Drive External Executive Alignment: Lead the resolution for high-priority client escalations at the CEO and Director level. You excel at taking a "stalled" implementation and using your executive presence to unblock the relationship, ensuring the provider sees the long-term value of the Stellar platform.

Own Data-Driven Operational Strategy: Define and track department-level KPIs (e.g., time-to-onboarding, user activation) to measure progress and report impact to senior leadership. Own the narrative of the implementation lifecycle. You will identify gaps in our current data infrastructure and build the systems necessary to track implementation performance -using these insights to tell a story to leadership about where we should invest resources next.

Nice to have:

VBC Exposure: You have a working knowledge of Value-Based Care models (e.g., ACOs, MSSP, Medicare Advantage). You don’t need to be a policy expert, but you understand how HEDIS scores and gap-closure impact a provider’s bottom line.

Systems Architecture: You have experience with the admin side of tools like Salesforce, Django, Tableau/Looker. You’ve personally helped design dashboards or workflows that help an implementation team stay organized.

Who will love this job:

You are a systems thinker - You don’t just see individual tasks; you see the gears that move the whole department. You get a genuine sense of satisfaction from taking a manual, 'bespoke' process and turning it into a streamlined, repeatable machine that doesn't break under pressure.

You are a workflow evangelist - You understand that in healthcare, the process is the product. You love the puzzle of aligning clinical reality with technical requirements, and you take pride in building workflows that feel seamless to a busy practice staff. You love the challenge of figuring out why a practice is struggling and finding the clinical or operational lever that makes the lightbulb go off for them.

You’re a "Player-Coach" - While you are a senior leader, you also love getting into the weeds with your team to help them solve a complex escalation. You also have the range to zoom out and present a 12-month strategy to our executive team.

You treat data as a strategic lever - You don’t just look at dashboards to see "what happened" - you use data to tell a story about what’s next. You’re someone who enjoys finding the signal in the noise and using those insights to influence other departments, advocate for your team’s priorities, and prove that our implementation strategy is actually moving the needle on provider performance.

A community builder - You understand that in a hybrid environment, culture requires intentionality. You naturally build community, celebrate wins, and bring an infectious energy that keeps the team motivated through the scaling journey.

Perks & Benefits:

Stellar offers a carefully curated selection of wellness benefits and perks to our employees:

Medical, Dental and Vision Benefits

Flexible PTO

Universal Paid Family Leave

Company sponsored One Medical memberships and Citibike memberships

Medical Travel Benefits

A monthly wellness stipend that gives employees the freedom to choose where they spend their cash, whether it be on wellness, pet care, childcare, WFH items, or charitable donations

Stock Options & a 401k matching program

Career development opportunities like Manager Training, coaching, and an internal mobility program

A broad calendar of company sponsored social events that for our in-office and remote employees

Diversity is the key to our success.
