This individual will dedicate 60% of their time performing the duties as a Nurse Practitioner and 40% of their time leading and supporting other Nurse Practitioners. (<em>This is subject to change based on the needs of the team and the company). </em>You will be responsible for ensuring the delivery of high-quality patient care making sure every healthcare interaction with Cadence is an exceptional experience that prioritizes the well-being of the patient and aligns with the goals of the health system.</p> <p><strong>The schedule for this role: Mon- Fri 9 am-6 pm MT or PT time zone. </strong></p> <p><strong>WHAT YOU’LL DO</strong>:</p> <p>Nurse Practitioner responsibilities:</p> <ul> <li>Supervise the tracking of patient vital signs, symptoms, and laboratory results in coordination with our Clinical Navigators and Registered Nurses (RNs)</li> <li>Coordinate the patient's treatment regimen and medications, focusing on starting and adjusting guideline-recommended medical therapy (GDMT) for Heart Failure, as well as other relevant guidelines for Type 2 Diabetes (T2D) and Hypertension.</li> <li>Address patient escalations  promptly, including abnormal vital signs, symptoms, and laboratory findings identified through Remote Patient Monitoring (RPM).</li> <li>Conduct regular virtual meetings with patients to discuss laboratory results and optimize adherence to GDMT</li> </ul> <p>Nurse Practitioner Supervisor responsibilities:</p> <ul> <li>Ensure adherence to clinical quality standards and compliance targets by conducting monthly audits, monitoring data for trends, and implementing performance enhancement strategies.</li> <li>Identify areas for quality improvement to enhance patient outcomes and satisfaction, and communicate objectives to the team, monitoring progress towards goals.</li> <li>Provide ongoing guidance, coaching, and mentoring to team members, fostering a collaborative and supportive environment conducive to teamwork and open communication.</li> <li>Assist in communicating policies, procedures, and care pathways, escalating matters to leadership when necessary, and collaborating with Team Leads to ensure adequate coverage.</li> <li>Manage staffing needs, approve PTO and CME, and complete time cards as required for the team.</li> <li>Participate in the recruitment process, interview prospective team members, and assist with onboarding and licensure.</li> <li>Stay updated on healthcare advancements and regulations, integrating them into organizational processes.</li> <li>Demonstrate flexibility to work across time zones to ensure team coverage.</li> <li>Ensuring every healthcare interaction with Cadence is an exceptional experience that prioritizes the well-being of the patient and aligns with the goals of the health system.</li> <li>Support internal growth efforts to help Cadence scale exceptional care delivery to patients with CHF and other chronic conditions including hypertension, Type 2 diabetes, and COPD.</li> <li>Be instrumental in shaping the culture of one of the fastest growing teams at Cadence.</li> </ul> <p><strong>WHAT YOU’LL NEED:</strong></p> <ul> <li>5+ years Nurse Practitioner experience treating patients with chronic diseases (T2D, Hypertension, CHF) either in an outpatient or inpatient setting at a high performing medical center.</li> <li>Master’s Degree as a Nurse Practitioner with willingness to expand state licensure as Cadence adds new markets and partners.</li> <li>5+ years of leading a clinical team NPs, RNs, and/or MAs. </li> <li>Board certification and active license required (ANCC/AANP). </li> <li>Multi-state compact RN licensure.</li> <li>Ability to thrive in an environment founded on trust, autonomy, and direct feedback.</li> <li>Excellent communication skills when leading and managing change initiatives.</li> <li>Analytical mindset with the ability to interpret data, derive actionable insights, and make data-driven decisions.</li> <li>Strong project management skills, with the ability to prioritize tasks and meet deadlines.</li> <li>Proven success in motivating and inspiring a clinical team.</li> <li>Experience coaching and training a team of clinicians.</li> <li>Passion for the patient / customer experience and systematically improving healthcare with digital innovation</li> <li>Prior experience working in a startup environment.
